Instructions

1. Purchase any bag of Dr. Elsey’s litter in-store at Target, Walmart, local small business pet store or online at Target.com or Walmart.com
2. Print and fill out this rebate form
3. Cut out the UPC/barcode from the product you purchased and mail it with the rebate form and original receipt (printed receipt is accepted for online orders) to the address on the form
4. Allow 8 to 10 weeks for delivery of your rebate
