Psst, this is a form from a rep from that MLM (pyramid scheme); if you do sign up, definitley use a throwaway account, or you'll be delluged with peope trying to get you to sell that stuff. While i loathe MLM's I think about this person is probably going to have their cult leader make them pay for each sample, and it is going to get so expensive, but they get excited and then a year from now they have a garage full of these samples....
