Offer Details

Any eligible person who completes the Survey by February 29, 2024 will receive a virtual Visa® prepaid card for $5. Participants must answer all questions and provide a complete name, mailing address, and email address in order to receive the virtual card. Incomplete Surveys will be discarded. There is a limit of 1 prepaid Visa card per person and e-mail address. Please allow 2-4 weeks for delivery of your virtual prepaid Visa card. Taxes, if applicable, are the responsibility of the participant.
