I am able to click the link and choose which sample I'd like, but free shipping is not an option. Since I don't want to leave my credit card, debit card, or use my PayPal, I'm not going to pay for shipping (even if it is only $1.00)

I didn't get a form, but it let me order a sample. The catch is you have to pay shipping. Find the item they're offering samples of and click the sample in the drop down section. You can get all 3 samples for the same shipping as you would pay for 1. It's only 1.00
