You need to order through a desktop or google chrome on your mobile device. I just got my box and it was full of goodies.

I had to switch to desktop after I got on the website. I had no problem, I got one for my stepdaughter. No price all free. Thank you

What should I do? A form is not even available for me?

This offer is actually not available anymore.

The box is free , all you do is sign up under the Welcome Baby box tab and click on where it says to sign up and you are then guided to the page where there is a Form to fill out with your name address and email. I did exactly that and after I filled out the form , The page will display you will receive your welcome baby box in 4 to 6 weeks. If you did not see a page saying you will receive your box in 4 to 6 weeks then you did not fill out the form correctly.

You can look up "Walmart baby welcome box sign up" and there's a link. The shipping isn't free though. It is $5.
