Once you click the link in our post above, you'll see a page that says "Enter your email below to register for your free Organic Valley drinking glass with purchase of $4 of Organic Valley milk." After you enter your email address, you will be prompted to enter your mailing address before you can access the upload form. If you have any questions, you can submit a request here and the administrator of the promotion (Prize Logic) will be able to assist you further.
